How to get started
Book your first appointment
Call us at (555) 234-5679 or request an appointment through the patient portal. Let us know you're a new patient so we can allow extra time for your first visit.
Complete your paperwork
You can fill out new patient forms online through the patient portal before your visit, or arrive 15 minutes early to complete them in the office.
Bring what you need
Insurance card, photo ID, a list of current medications (including dosages), and any questions for your provider.
Meet your provider
Your first visit is a chance to share your health history, discuss any concerns, and start building a relationship with your care team.
What to expect
Your first visit typically takes 45–60 minutes. Your provider will review your medical history, perform a basic health assessment, and discuss any immediate concerns or goals.
This is your time. Ask questions, share worries, and let us know what matters most to you. There's no concern too small.
